Anthem BC PPO加入
From Alison: You are on the Platinum plan. CME list is the right document you sent me… the Summary of Benefits and Coverage.Office Visits are $20 for a general doctor and $40 for a specialist… like a nephrologist.
You don’t need a referral from a CA doctor to see a doctor in another state. In fact, with a PPO plan, you don’t EVER need a referral to a specialist, regardless off his/her location. However, when you use non-CA providers you want to make sure that the doctors are in either the Anthem BC network and/or the Blue Shield network. The IN-network benefit for ANY in-network provider – doctor or hospital – is 90% paid by Anthem BC and 10% contributed by you. Your 10% contribution has a maximum of only $4000… then the insurance company pays 100% of all remaining and eligible charges for the year.
